Getting home from a BUPA care home

Greta had a fall and fractured right neck of femur and following her hospital discharge she was residing in a care home for over 6 months. Her husband contacted Home Independence Services as they were concerned about the length of time she was residing there and the need to get her home. Our Occupational Therapist visited her in the Care Home to complete an assessment and also visited the family home to work out what equipment and adaptations would assist her safe return home.

Our Occupational Therapist prescribed a number of items of equipment to help her with activities of daily living including a bed lever, grab rails for the bathroom, toileting equipment, and further rails outside.

Following Greta's return home a live in carer was brought in to help and so Home Independence Services returned to provide further training and support to ensure safe transfers and best techniques for safe handling were being deployed.
